UPPSC has released the admit card for the Swasthya Shiksha Adhikari (Screening) Exam-2025. It went live on 1 October 2026 and you download it from uppsc.up.nic.in with your OTR number, date of birth and gender. The download stays open till 4 October 2026. Swasthya Shiksha Adhikari (स्वास्थ्य शिक्षा अधिकारी) means Health Education Officer. UPPSC's notice number for this exam uses the short form HEO.
The exam is on Sunday, 4 October 2026, in one session from 9:30 AM to 11:30 AM, in 10 districts of Uttar Pradesh. It is the screening test for 221 posts in the Directorate General of Family Welfare, Uttar Pradesh (Advertisement No. D-6/E-1/2025, Department No. S-06/03).

UPPSC's direct links to its admit card pages don't work when you click them from this site or paste them into your browser. They open the UPPSC home page instead. So start from the home page and follow these steps.
The page gives an admit card only if your application was not rejected and your exam has been scheduled. The same login page also has "Know your Registration Number", "Know your Roll Number" and "View Application Status" links. If you don't know your OTR number, use Know your OTR number in the Candidate's Help Desk Section.
District slip vs admit card: since 28 September 2026, UPPSC has also offered an exam district information slip. To get it, click Download Examination - District Information Slip in the Candidate's Help Desk Section, then DOWNLOAD EXAM DISTRICT SLIP in the Swasthya Shiksha Adhikari row, and log in with the same details. It is advance information of the district where your exam centre is. UPPSC says it is not an admit card for the exam.

Under the Recruitment Dashboard tab, click the admit card link for ADVT.NO.D-6/E-1/2025, SWASTHYA SHIKSHA ADHIKARI (or click Download Admit Card in the Candidate's Help Desk Section). Click DOWNLOAD ADMIT CARD in the Swasthya Shiksha Adhikari row, then enter your OTR number, date of birth, gender and the verification code.

Along with your admit card, UPPSC says you must bring:
Timing: entry to the centre starts at 8:00 AM (1 hour 30 minutes before the exam) and closes at 8:45 AM (45 minutes before the exam). The paper runs from 9:30 AM to 11:30 AM.
OMR sheet: answers are marked on an OMR sheet by blackening the circles. Don't use whitener, a blade, a pin or an eraser on it. UPPSC does not evaluate an OMR sheet where circles are not blackened properly or details are filled wrongly.
Unfair means: the Uttar Pradesh Public Examinations (Prevention of Unfair Means) Act, 2024 applies to this exam. Under this Act, using unfair means in the exam, copying or helping someone copy, reproducing or leaking the question paper, or conspiring to leak it are offences. In such cases, a fine of up to ₹1 crore and imprisonment up to life can both be imposed.

Part I, General Studies: History of India and the national movement; Indian and world geography (with Uttar Pradesh); Indian polity and Constitution (with the political system of UP); Indian society and economy, including health, disease and sanitation; current affairs of national and international importance, with sports; Indian agriculture; general science up to class 10; elementary maths up to class 10; and community medicine (National Health Mission, national health programmes, health education and behaviour change communication, community and resource mobilisation).
Part II, Social Science Related Knowledge: Sociology, Anthropology, Psychology, Social Psychology, Political Science, Geography, History and Economics. The topics include social factors of health, public health, community health, health geography, health governance and healthcare infrastructure.
